Our Policy on Gathering Information
In the course of providing our services to you as our customer or to members of your family, we may obtain significant private personal financial and health information consisting of:
· Personal contact information, including name, residential and business mailing addresses, telephone numbers, e-mail addresses, and Social Security numbers. · Substantive information from interaction with us, including correspondence by mail or electronic mail, facsimile transmissions, telephone conversations, and personal meetings. · Information about personal finances, including bank accounts, investments, debts and liabilities, insurance policies and claims, tax information, and holdings in personal and real property. · Information about personal health and healthcare including diagnosis, prognosis, treatment, healthcare providers and reports. · Any other information which may be provided to or shared with us in order to help us represent your interests more effectively.
Our Policy on Disclosing Information
Whether during the term or our active representation, or after our services on your behalf have been terminated and you cease to be our customer, our policy is the same. We do not disclose private personal information concerning you or members of your family or relating to our representation and services to anyone except as specifically authorized or reasonably required for us to perform our contract services, or as otherwise required by law.
Our Policy on Access and Security of Information
We retain complete records relating to the information we obtain and the services we provide. In order to protect your information we maintain physical, electronic, and procedural safeguards consistent with appropriate privacy policies, standards and guidelines to protect against the loss, unauthorized access or disclosure of your private information.
We retain all accounting files for a period of seven years and all medical claims files for a period of four years, after which they are destroyed without further notice. You must notify our office should you want the files returned to you or your representative. |
